Abstract: Kecelakaan kerja pada proyek konstruksi, terutama dalam aktivitas berisiko tinggi seperti penurunan material dan pembongkaran scaffolding, mengakibatkan kerugian finansial dan produktivitas. Meskipun ada upaya pencegahan, kecelakaan tetap terjadi karena penyebab yang kompleks. Penelitian ini bertujuan mengidentifikasi kecelakaankerjaberisiko tinggi pada pembangunan pabrik PT. Agri First Indonesia, menganalisis penyebabnya denganFault Tree Analysis (FTA), dan merumuskan strategimitigasi. Pendekatan FTA top-down digunakan untuk memetakan hubungan hierarkis antara kecelakaan dan factor penyebabnya (personal, manajemen, teknis, lingkungan). Data diambil dari laporan bulanan dan divisualisasikan dalam model grafis FTA. Tujuhrisikokecelakaan kategori tinggi teridentifikasi, dengan penurunan material dan scaffolding sebagai yang paling dominan. Penyebab utamanya meliputi human error, pengawasan lemah, alat rusak, dan cuaca ekstrem. FTA mengungkap 23 basic event, 12 di antaranyaterkait faktor personal/manajemen. Studi ini merekomendasikan solusi berbasis teknologi (IoT, BIM), pelatihan intensif, dan penerapan standar K3 internasional (OSHA/ISO 45001). Implementasi holistik dapat menurunkan kecelakaan hingga 40%. Occupational accidents in construction projects, especially in high-risk activities such as material dropping and scaffolding dismantling, result in financial and productivity losses. Despite prevention efforts, accidents still occur due to complex causes. This study aims to identify high-risk occupational accidents in the construction of PT. Agri First Indonesia's factory, analyze the causes using Fault Tree Analysis (FTA), and formulate mitigation strategies. The top-down FTA approach is used to map the hierarchical relationship between accidents and their causal factors (personal, management, technical, environmental). Data are taken from monthly reports and visualized in a FTA graphical model. Seven high-risk accidents were identified, with material dropping and scaffolding as the most dominant. The main causes include human error, poor supervision, broken tools, and extreme weather. FTA revealed 23 basic events, 12 of which were related to personal/management factors. This study recommends technology-based solutions (IoT, BIM), intensive training, and the implementation of international OHS standards (OSHA/ISO 45001). Holistic implementation can reduce accidents by up to 40%.
