Rakastore is an MSME that produces MDF Posters. The problem currently being faced by Rakastore Medan is that defects in MDF posters exceed 20% of total production each month. Based on these conditions, it is necessary to resolve the causes of defects in MDF posters using the Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methods, which aim to determine the type of defect, determine the factors that cause the defect and provide suggestions for improvement. Based on the results of the Cause and Effect Diagram and FMEA analysis that has been carried out, it is known that there are 4 types of defects, namely: scratched, curved posters, inappropriate size and inappropriate print color. The highest Risk Priority Number (RPN) value is 648 for defects caused by machines. The print is damaged and adjustments have not been made correctly for incorrect color defects. It is necessary to schedule machine maintenance, improve SOPs, provide training and direction before carrying out work and carry out cleaning activities before and after activities so that the work environment is clean, neat, comfortable and safe. The response to the cause of this failure is continuous maintenance. | en_US |
