The purpose of this study was to identify factors that influence relapse in survivors of addictive substances at the Medan Plus Rehabilitation Center. The research sample consisted of 30 male people aged 20-50 years. The sampling technique used the total population. Data collection used a rating scale based on relapse factors according to Barati (2020). The results of the rating scale analysis showed that the 3 highest dominant factors influencing relapse were ranked based on their contribution, namely the factor of friends who are addicted 5.23, the factor of temptation 5.53, and the factor of disputes between families 5.93. The results of the analysis showed that the social environment was the main factor influencing relapse in respondents, as well as the influence of temptation, and disputes between families. The findings obtained were that they experienced relapse less than 6 months after leaving the rehabilitation center. These results as a whole show what survivors, families, communities and the government need to do to anticipate survivors from returning to drug use after initial rehabilitation (relapse).en_US
