dc.description.abstractPenelitian ini bertujuan untuk menganalisis penyebab terjadinya kerusakan paket (package damage) dan memberikan usulan perbaikan kualitas layanan pada jasa logistik PT. Indah Express dengan menggunakan metode PDCA (Plan-Do-Check- Act) dan FMEA (Failure Mode and Effect Analysis). Data kerusakan paket yang dianalisis berasal dari periode Juli hingga Desember 2024, di mana ditemukan total 5.066 paket rusak dari 61.187 paket yang dikirim, dengan rata-rata persentase kerusakan sebesar 8,3% per bulan, melebihi standar perusahaan yang ditetapkan sebesar 5%. Tingkat kerusakan tertinggi terjadi pada Oktober 2024, yaitu 1.032 paket rusak dari 10.234 paket (10,1%). Penelitian ini menggunakan pendekatan deskriptif dengan pengumpulan data kerusakan paket, analisis sebab-akibat menggunakan diagram fishbone, brainstorming tim, serta penilaian risiko menggunakan FMEA untuk menentukan prioritas perbaikan. Analisis menggunakan diagram sebab-akibat (fishbone diagram) dan FMEA menunjukkan bahwa faktor utama penyebab kerusakan adalah kesalahan dalam proses pengemasan, penanganan, dan pengiriman. Melalui implementasi metode PDCA, dilakukan perencanaan, pelaksanaan, pemeriksaan, dan tindakan korektif berupa pelatihan ulang karyawan, perbaikan prosedur pengemasan, serta peningkatan pengawasan pada proses pengiriman. Hasil penelitian menunjukkan bahwa penerapan perbaikan berbasis PDCA dan FMEA berpotensi menurunkan tingkat kerusakan paket dan meningkatkan kualitas layanan logistik di PT. Indah Express. Temuan ini diharapkan dapat menjadi acuan bagi perusahaan dalam upaya peningkatan mutu pelayanan serta pencegahan kerusakan paket di masa mendatang. This study aims to analyze the causes of package damage and propose quality improvement measures for the logistics services of PT. Indah Express using the PDCA (Plan-Do-Check-Act) and FMEA (Failure Mode and Effect Analysis) methods. The analyzed package damage data covers the period from July to December 2024, during which a total of 5,066 damaged packages were recorded out of 61,187 packages shipped, resulting in an average monthly damage rate of 8.3%, which exceeds the company’s standard threshold of 5%. The highest damage rate occurred in October 2024, with 1,032 damaged packages out of 10,234 shipments (10.1%). This research uses a descriptive approach, involving the collection of package damage data, cause-and-effect analysis using a fishbone diagram, team brainstorming, and risk assessment through FMEA to determine repair priorities. Analysis using the fishbone diagram and FMEA indicates that the main factors causing damage are errors in the packaging, handling, and delivery processes. Through the implementation of the PDCA method, planning, execution, inspection, and corrective actions were carried out, including employee retraining, improvement of packaging procedures, and enhanced supervision of the delivery process. The study results show that implementing improvements based on PDCA and FMEA has the potential to reduce package damage rates and improve the quality of logistics services at PT. Indah Express. These findings are expected to serve as a reference for the company in its efforts to enhance service quality and prevent future package damage.en_US
