CV. Hamparan Sawit Makmur is a company engaged in the commodity of agricultural producis that focuses on crude palm oil products (Crude Palm Oil)and Kernels. Based on the results ofinterviews and direct observations in the field,it turns out thar there have been accidents and there are other potential hazards that may occur at each work station. Where in the field there are risk factors that are classified as unsafe acts and unsafe situations and the lack ofa role in occupational safety management in the Company. The purpose ofthe study is to identifyeveryexisting risk, both that has occurred and the risk that mayoccur and will be sorted based on the effect ofhazards, the chance of occurrence, early control, and cost estimates using the Multi Attribute Failure Mode Analysis (MAFMA) method. There were 17 validated risk events. The results ofthe analysis using the Multi Attribute Failure Mode Analysis (MAFMA) method obtained some ofthe most influential risks at each work station, namelyexposure to vehicle smoke of0.108, slippery shoe soles of0.091, contact with electricity of0.85, pinched v-belt chain coupling of0.85, and respiratorydisorders of0.108 Total risk.en_US
